A modifier changes the same key while you hold Shift, Ctrl or Alt. WoW reads conditions from left to right and uses the first valid choice, while the final unconditioned spell is the normal press. This is useful for pairing related abilities, choosing a target or making aura cancellation deliberate.
Use them when two related actions belong on one comfortable key, when Alt should force a self-cast, or when Shift should target your focus. Avoid them when the key is already bound elsewhere, when you regularly press the wrong modifier under pressure, or when separate buttons are clearer. They never bypass the global cooldown and cannot make combat decisions.
A focus is simply a saved secondary unit. It is useful for interrupting an assigned add, helping the same tank or using utility on one player without changing your main target. You can play every class without focus macros; use them only when they make your own raid role easier.
Grid2 and custom raid frames work well with mouseover macros. Clique, HealBot and VuhDo can bind spells directly to frame clicks, so separate mouseover or focus buttons may become unnecessary. Clear nameplates can make hostile mouseover macros faster, while reliable boss frames can reduce the need to store a boss as focus. Build around the interface you actually use.
Normal press: Hand of Salvation. - Shift + press: Hand of Freedom. The same action-bar button now contains two related abilities. The three standard modifiers are:

The result is: 1. Shift casts Spell A. 2. Ctrl casts Spell B. 3. No listed modifier casts Spell C. The final spell without a condition is the default action. You can write that default explicitly with [nomod]:

[nomod] means that Shift, Ctrl and Alt must not be held.

This works as follows: 1. Alt casts Hand of Sacrifice on yourself. 2. Shift casts it on your focus. 3. A valid friendly mouseover receives it. 4. Otherwise, the spell uses normal targeting. Another common healing pattern is:

Alt always heals yourself. - Otherwise, it heals your mouseover. - If there is no mouseover, it heals your friendly target. - If neither exists, it heals you.

Normal press activates Divine Shield. - Shift + press removes Divine Shield. This is safer than a macro that always activates or cancels the aura, especially for players who repeatedly press their keybinds. The same idea can be used with Divine Sacrifice:

Normal press activates Divine Sacrifice. Alt allows controlled cancellation of its aura. The exact Divine Sacrifice and Divine Guardian interaction should be tested on the server where the macro will be used.

Normal press: Seal of Vengeance. - Shift: Seal of Command. A Horde Paladin would replace Seal of Vengeance with Seal of Corruption. Another example:

Normal press: Fire Ward. - Shift: Frost Ward. This is useful because both abilities serve a similar purpose and are unlikely to be needed simultaneously.

If Shift+1 is already assigned to another action-bar slot, the existing Shift+1 keybind may take priority instead of the macro receiving the modifier. Check the keybind menu and remove conflicting bindings. Alt may also conflict with the game’s self-cast setting. If Alt is configured as the automatic self-cast modifier, decide whether to keep that behavior or use Shift/Ctrl in your macros instead.

If Spell A activates the global cooldown, Spell B normally cannot fire from the same press. Items and abilities outside the global cooldown can sometimes be combined:

This may activate engineering gloves and Avenging Wrath together because the glove enchant does not use the normal spell global cooldown.

This prioritizes your mouseover and otherwise uses your selected target. Clear it with:

An ability can then be sent to the focus:

If the focus dies, disappears or was never set, nothing happens. A safer version falls back to the current target:

The empty [] means “use normal casting behavior.” This gives the following priority: 1. Valid hostile focus. 2. Current target. For friendly abilities:

If your cursor is over a Grid2 unit frame, the spell is cast on that player. In this setup, a dedicated focus macro may be unnecessary for frequently changing friendly targets.

The player can apply the spell through an enemy nameplate without changing targets. If the nameplates are clear and reliable, mouseover interrupts or multidot macros may feel better than focus macros.
